Acute Pancreatitis (Necrotizing Pancreatitis) – Acute pancreatitis or infection in the pancreas can sometimes lead to necrotizing pancreatitis if left untreated. In necrotizing pancreatitis, the part of the pancreas starts dying and stops functioning. This tissue death leads to the development of diabetes mellitus as well. The most common symptom is abdominal pain at the front that goes to the back as well. Fever, nausea, and vomiting may or may not accompany. Gall stones and alcohol are the main causes of pancreatitis. First of all acute pancreatitis is treated by giving antibiotics and other supportive medications. After that, the necrotizing or the dead piece of the pancreas is removed. Most patients make the full recovery.
Pseudocyst Of Pancreas – The pseudocyst of the pancreas is a pouch or sac-like structure that is filled with pancreatic enzymes, dead tissue, and blood. It is most often caused as a complication of chronic pancreatitis. Abdominal pain, jaundice, nausea, and vomiting are the presenting complaints. CT scans and pancreatic enzyme tests are performed to find out the presence of pseudocyst. These pseudocysts need to be drained if they are growing large and are causing symptoms. These cysts are drained either endoscopically or by surgical intervention. A connection of pseudocyst is made with the nearby organ so that it can be drained via that organ.
Chronic Pancreatitis – The infection in the pancreas that failed to improve turns into chronic pancreatitis. Abdominal pain, high blood sugar levels, indigestion, belching, gastric problems, and clay-colored stools are the signs of chronic pancreatitis. The diagnosis is made by ultrasound, MRCP, CT-scan, or MRI procedures. Treatment of chronic pancreatitis includes pain-relieving medications, anti-oxidants, and enzyme tablets to help in digestion. In severe cases, surgical interventions such as pancreatic surgery and Whipple procedure are performed.
Cancer Of The Pancreas – Pancreatic cancer is a silent disease that develops no symptoms in the early stages. The symptoms in the late stages of cancer are abdominal pain that may travel to the back, weight loss, loss of appetite, jaundice, nausea, and fatigue, etc. The pancreatic tumor is removed surgically (Whipple procedure), followed by chemo and radiotherapy. In severe cases, the whole pancreas needs to be removed by distal or total pancreatectomy. Neuroendocrine Tumours – Neuroendocrine tumors grow anywhere in the digestive organs such as the digestive tract, pancreas, and rectum, etc. Most of them show no symptoms and are detected incidentally. They can be benign or cancerous as well. Fluctuating blood sugar levels and low blood sugar is often seen in such patient who show some symptoms. The tumor is removed surgically, most of the time it is present in the pancreas so pancreatectomy (removal of a part or whole pancreas) may be required as well. Chemotherapy and radiotherapy follow as per the patient’s health and if the tumor is cancerous.
Pancreatic Cystic Neoplasms – These are the cysts present in the pancreas that can be cancerous or non-cancerous. These cysts are detected accidentally as they produce no symptoms in most cases. These cysts are detected in a CT scan or MRI accidentally and then an endoscopic ultrasound is performed to detect them closely. A needle is passed during the endoscopy procedure to check the cells in the cyst. This sample is then sent to the lab to find out the presence of cancerous cells if any. Some cysts don’t have cancerous cells but have mucin in them, such cysts are considered pre-cancerous and need to be watched closely or removed. Surgical removal of cancerous cysts is performed for permanent treatment.
Other Benign Tumors Of Pancreas – Many other benign tumors of the pancreas are closely watched and observed for any symptoms. Their growth is measured and any alarming signs are noted to remove them whenever required. These benign tumors may hamper insulin production and create digestive problems depending upon their size and location in the pancreas. Whipple’s Pancreaticoduodenectomy – Whipple procedure or pancreaticoduodenectomy is the major surgical procedure for removal of tumor over the pancreas. It is used for the treatment of pancreatic cancer. In the whipple procedure, the head of pancreas, gallbladder, some part of intestine (duodenum), a part of bile duct and the lymph nodes around it. This is also a complex procedure that Dr. Jha has performed many times in the past and is an important procedure to preserve function as well as remove the tumor. The patient is discharged after a week and can resume normal activity after 6 to 8 weeks. Chemotherapy and radiotherapy may or may not follow, depending upon the extent of cancer spread and stage of tumor.
Frey’s procedure – Frey’s procedure is performed in case of an old inflammation in the pancreas, known as chronic pancreatitis. In this procedure, the diseased or inflamed portion of the head of the pancreas is removed. This procedure provides better drainage of the ducts and the treatment of this infection as well. After the removal of the diseased portion, the pancreatic ducts drain more efficiently into the remaining healthy portion of the pancreatic head.
Distal Pancreatectomy – In this surgery, the distal portion of the pancreas that is the body and the tail are removed. This procedure is performed to remove a tumor present in the distal portions of the pancreas. In some cases, the spleen is also removed if the tumor has spread to that as well. This surgery takes about 2 to 3 hours and the pancreas is still able to function.
